If DOR advises you to report ownership changes to our office, please file an annual report to report these changes.
Under the Controlling Interest section, question 2 asks, "In the past 12 months, has there been a transfer of at least 16 ⅔ percent of the ownership, stock, or other financial interest in the entity?"
If you are not within 180 days of your annual report due date, please file an Amended Annual Report.
